On September 18, 2025, a U.S. media outlet reported that Tesla's Full Self-Driving (FSD) system has come under fresh scrutiny, leading to an investigation by U.S. regulatory bodies. This development arose from an incident in June of the same year when Tesla owner Italo Frigoli was driving with the FSD feature engaged. As his vehicle neared a railway crossing, the system failed to identify the descending barriers and an oncoming train. This compelled Frigoli to abruptly apply the brakes to avert a potential collision. Recently, when Frigoli traversed the same crossing again, the FSD system once more failed to detect the train, necessitating another manual intervention to stop the vehicle.
Furthermore, six additional Tesla FSD users have reported encountering similar technical malfunctions. Among these, four users have provided video footage as evidence. From June 2023 to August 2025, seven more videos surfaced online, all depicting analogous failures of Tesla's FSD system at railway crossings. The U.S. National Highway Traffic Safety Administration (NHTSA) has indicated that it has been in communication with Tesla and will persist in scrutinizing consumer complaints to ascertain whether there is an emerging pattern of vehicle safety defects.
