The United States National Transportation Safety Board (NTSB) has concluded that the cabin door plug detachment incident on a Boeing 737 MAX 9 aircraft in 2024 was primarily attributed to insufficient employee training and guidance in the manufacturing process, coupled with inadequate oversight by the Federal Aviation Administration (FAA). The investigation unveiled that during the assembly and maintenance of the aircraft, the four bolts securing the door plug were removed but were not reinstalled, exposing systemic deficiencies in Boeing's manufacturing procedures and oversight mechanisms of regulatory bodies.
