The Occupational Safety and Health Administration (OSHA) determined that SpaceX failed to conduct a standard inspection on a recently repaired hydraulic crane prior to a crane collapse incident at its Starbase facility in Texas. As a result, the company was fined $115,850 for seven serious violations. Previously, the injury rate at the Starbase facility far exceeded industry standards, prompting OSHA to require SpaceX to implement multiple safety improvements.
